They work by utilizing an outdoor unit and an indoor unit (you’ll hear central systems referred to as split systems). Heat is drawn into the system from your home, and then that air going through a refrigerant cycle to provide you with cooled air through your vents.
There have been a number of advancements made in recent years to these systems, including variable speed technology to make them even more efficient. So long as your central air conditioner is well cared for through routine maintenance and timely repairs, it will last you a very long time.
